Full Job Description

The ideal candidate will be able to demonstrate through experience, their ability to undertake comprehensive patient focussed medication reviews for patients within Primary care. They will need to demonstrate the ability to work autonomously and as part of a wide multi-disciplinary team. The role will involve promoting and supporting the delivery of safe, cost-effective, evidenced based prescribing across Primary Care., The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

An excellent opportunity for an enthusiastic, adaptable and highly motivated pharmacist has arisen at Swansea Bay University Heath Board, to maximise safe and effective care for patients across primary care.
You will have the opportunity to apply your expert pharmaceutical knowledge and skills to improve patient care through a range of settings including GP practice, virtual wards, care homes etc.
Primary care experience is desirable but we would welcome applications from those with hospital or community experience who are able to clearly demonstrate the aptitude required to work within a busy changing environment., We are looking for applications from enthusiastic, self-motivated clinical pharmacists who are effective communicators with a passion to provide evidenced based patient care.

Please be advised that there is a temporary top up for Bands 1,2 and 3 to reflect the incorporation of the top up to the living wage of £12 per hour - £23,465 per annum. This temporary top up will be in place until the annual pay uplift for 2024/25 is confirmed.
