Full Job Description

  • Work closely with the Nurse Manager Team to provide quality care to an adult client receiving end of life care at home

  • Commit to a regular working pattern and collaborate with Care Coordinators for effective care delivery.

  • Provide 2-2-1 support to an an adult carrying out observations

  • Build rapport with clients and form a part of their daily routine

  • Potential for growth and development into a supervisor or management role.
